What It Is
Semaglutide is a GLP-1 receptor agonist. It is a 31-amino acid peptide that mimics a hormone your gut naturally releases after you eat. It was originally developed for type 2 diabetes and is now one of the most studied compounds for fat loss in medical history.
What It Does
Semaglutide reduces appetite, slows how fast food leaves your stomach, lowers blood sugar, and improves insulin sensitivity. In clinical trials, patients lost an average of 15 to 17 percent of their body weight over 68 weeks. It also reduces cardiovascular risk and improves metabolic markers across the board.
How It Works
It binds to GLP-1 receptors in the brain, gut, and pancreas. In the brain, it reduces hunger signals and increases the feeling of fullness. In the gut, it slows digestion so you feel satisfied longer. In the pancreas, it improves insulin release in response to food.
Who Benefits Most
Best for people with significant weight to lose, insulin resistance, metabolic syndrome, or type 2 diabetes. Also useful for people who have tried everything and cannot get the scale to move. Works best when combined with nutrition and lifestyle support.
Protocol and Dosing
Typical starting dose: 0.25 mg once weekly. Titrate up every 4 weeks based on tolerance. Maintenance dose: 1 to 2.4 mg weekly. Cycle length: 16 to 52 weeks depending on goals.
| Vial Format | Concentration | 0.25 mg dose | 0.5 mg dose | 1 mg dose |
|---|---|---|---|---|
| 5 mg/mL | 5 mg/mL | 5 units / 0.05 mL | 10 units / 0.1 mL | 20 units / 0.2 mL |
Cautions
Not for use in people with a personal or family history of medullary thyroid cancer or MEN2. May cause nausea, vomiting, or constipation, especially at higher doses. Start low and titrate slowly. Not recommended during pregnancy.
Pairs Well With
Tirzepatide is a stronger option if you want dual GIP and GLP-1 action. Retatrutide is the most aggressive metabolic option, adding GIP, GLP-1, and glucagon receptor activity. BPC-157 can help with GI side effects during the titration phase.
